We appreciate your time and commitment to Black students pursuing a health care career to strengthen the nation's health by improving the health care workforce's diversity. We rely on dedicated volunteers to help us review scholarship applications. The scholarship review committee is your chance to do your part for the future of health care in our community.
Alexis G. Philius Scholarship applications will be reviewed this spring. Volunteers will receive scholarships to review via email, with grading being entered online.
Statement of Commitment: This form acknowledges my commitment to serve as a volunteer on the 2021 Alexis G. Philius Scholarship Review Committee. You may be asked to review up to 20 short essays.
Due Date: All scoring will need to be completed within two weeks of receipt.
I understand that I am volunteering on this committee, and I will not receive any compensation.
Statement of Confidentiality: Information contained in the scholarship application forms, essays, and transcripts must be maintained in a confidential manner at all times. Distribution and/or reproduction of any application forms, essays, transcripts, and other scholarship documents outside the intended and approved use is strictly prohibited. All application forms and essays shall be destroyed and/or returned to UNAC/UHCP.
