Study Background and Details
Dr. Ann Mayo, a faculty member in the School of Nursing at the University of San Diego, San Diego, CA, and Denise Duncan, president of United Nurses Association of California/Union of Health Care Professionals (UNAC/UHCP), are conducting a research study about nurse perceptions of medication errors and we would like you to participate.
This study is endorsed by United Nurses Association of California/Union of Health Care Professionals (UNAC/UHCP), Connecticut Health Care Associates (CHCA District 1199 NUHHCE), AFT Connecticut (AFT), Hawaii Nurses and Healthcare Professionals (HNHP), National Union of Hospital and Health Care Employees 1199 J (NUHHCE 1199 J), Oregon Federation of Nurses and Health Professionals (OFNHP), and United Food & Commercial Workers (UFCW).
Purpose of study
The purpose of this study is to understand what nurses think about medication errors and the reporting of medication errors. You are being asked to participate because you are a practicing nurse.
Description of survey
If you participate, you will be asked to complete an online survey once that takes about 15 minutes to complete. You will be asked questions such as: Given a list of items, what on the list do you think cause medication errors? What percent of medication errors do you think are reported?
You will also be asked a few questions about yourself, such as your age, years of practice, if you are an RN, LVN/LPN or advanced practice nurse.
Confidentiality of survey
This study involves no more risk than the risks you encounter in daily life. Your responses will be kept confidential and all your information will be coded with a number. Your email or IP address will automatically be deleted, and no one will know your identity. Ann Mayo will keep the study data for a minimum of 5 years.
Taking part in this study is entirely optional. Choosing not to participate will have no effect on your employment status or any other benefits to which you are entitled. You may also quit being in the study at any time or decide not to answer any specific questions. Should you decide to participate, please print out a copy of this page for future reference.
